S, TlCiA BMSCII, City C PROVE'D AS TO FORM: C I, Attornev ACIfO kANllRU,4 Slk_ Exhibit "A" AGREEMENT TO EXTEND TERM STATE OF TEXAS COUNTY OF HARRIS This Agreement to Extend Term is effective as of January 1, 2011, (the "Extension Effective Date ") and extends that certain Purchased Services Agreement between Jacinto Medical Group, P.A., a Texas professional association ( "Provider ") and the City of Baytown, a Texas home -rule municipal corporation (the "City") dated and effective January 1, 2010 (the "Agreement "). WHEREAS, City and Provider entered into the Agreement to detail the responsibilities of each party in the Provider's provision of medical services to City employees and certain family members at Provider's Occupational Health Services Clinic; and WHEREAS, the parties desire to extend the term of the Agreement until March 31, 2011, at which time the parties may agree to extend the term of the Agreement again pursuant to Section 3.a of the Agreement; NOW, THEREFORE, in consideration of the mutual covenants and conditions, Provider and City hereby extend the term of the Agreement in accordance with Section 3.a of the Agreement through March 31, 2011, under the same terms, conditions, and compensation as established therein, which compensation shall be one installment of THIRTY THOUSAND AND NO /100 DOLLARS ($30,000.00).
