AN ORDINANCE OF THE CITY COUNCIL OF THE CITY OF BAYTOWN, TEXAS,
RENEWING THE ADMINISTRATIVE SERVICES AGREEMENT WITH
OPTUMHEALTI -I BEHAVIORAL SOLUTIONS, F /h/A UNITED BEHAVIORAL
HEALTI I FOR THE CITY'S EMPLOYEE ASSISTANCE PROGRAM: AUTHORIZING
PAYMENT BY THE CITY OF BAYTOWN. THE SUM OF TWENTY -SIX THOUSAND
SEVEN HUNDRED FIFTY AND NO /100 DOLLARS ($26,750.00); AND PROVIDING
FOR THE EFFECTIVE DATE THEREOF.
13E IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF BAYTOWN, "I'EXAS:
Section l: That the City Council of the City of Baytown, Texas. hereby renews the
Administrative Services Agreement with Optuml- lealth Behavioral Solutions f/k/a United Behavioral Health
for the City's Employee Assistance Program under the same terms and conditions.
Section 2: That the City Council of the City of Baytown authorizes payment to OptumHealth
Behavioral Solutions f/k/a United Behavioral Health, of the sum of TWENTY -SIX THOUSAND SEVEN
HUNDRED FIFTY AND NO/] 00 DOLLARS ($26,750.00), pursuant to the Agreement.
Section 3: That the City Manager is hereby granted general authority to approve any change
order involving a decrease or an increase in costs ofTWENTY -FIVE TI IOUSAND AND NO/ 100 DOLLARS
($25,000.00) or less: however, the original contract price may riot be increased by more than twenty-five
percent (25 %) or decreased by more than twenty -five percent (25 %) without the consent of the contractor to
such decrease.
Section 4: This ordinance shall take effect immediately from and after its passage by the City
Council of the City of Baytown.
