Ordinance No. 9,625ORDINANCE NO. 9625 AN ORDINANCE OF THE CITY COUNCIL OF THE CITY OF BAYTOWN, TEXAS, AUTHORIZING THE CITY MANAGER TO EXECUTE AN AGREEMENT WITH UNITED HEALTHCARE OF TEXAS, INC., TO SERVE AS A THIRD PARTY ADMINISTRATOR FOR THE ADMINISTRATION OF MEDICAL AND DENTAL CLAIMS AND FOR NETWORK SERVICES ON BEHALF OF THE CITY OF BAYTOWN, AUTHORIZING THE CITY MANAGER TO APPROVE THE SPECIFIC AND AGGREGATE STOP LOSS COVERAGE UNITED HEALTHCARE OF TEXAS, INC.; AUTHORIZING PAYMENT OF SEVEN HUNDRED FIFTY -TWO THOUSAND NINE HUNDRED EIGHTY -FOUR AND NO/] 00 DOLLARS ($752,984.00); AND PROVIDING FOR THE EFFECTIVE DATE THEREOF. BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF BAYTOWN, TEXAS: Section 1: That the City Council of the City of Baytown, Texas, hereby authorizes the City Manager to execute an agreement with United HealthCare of Texas, Inc., to serve as the third party administrator for the administration of medical and dental claims and for network services on behalf of the City of Baytown under terms and conditions deemed acceptable to the City Attorney and the City Manager. Section 2: That the City Council of the City of Baytown authorizes payment to United HealthCare of Texas, Inc., of THREE HUNDRED FIFTY -FIVE THOUSAND EIGHTY -THREE AND NO 1100 DOLLARS ($355,083.00), pursuant to the agreement identified in Section 1 hereof. Section 3: That the City Council of the City of Baytown, Texas, hereby authorizes the City Manager to approve the specific and aggregate stop loss coverage with United HealthCare of Texas, Inc., under terms and conditions deemed acceptable to the City Attorney and the City Manager. Section 4: That the City Council of the City of Baytown authorizes payment to United HealthCare of Texas, Inc., of THREE HUNDRED NINETY -SEVEN THOUSAND NINE HUNDRED ONE AND NO /100 DOLLARS ($397,901.00), for the coverage identified in Section 3 hereof. Section 5: That the City Manager is hereby granted general authority to approve any change order involving a decrease or an increase in costs of TWENTY -FIVE THOUSAND AND NO /100 DOLLARS ($25,000.00) or less; however, the original contract price may not be increased by more than twenty -five percent (25 %) or decreased by more than twenty -five percent (25 %) without the consent of the contractor to such decrease. n LJ 0 Section 6: This ordinance shall take effect immediately from and after its passage bythe City Council of the City of Baytown. • INTRODUCED, READ and PASSED by the affirmative vote of the City Counci 1 of the City of Baytown this the 11`h day of September, 2003. , xo- C. tq"--a- PETE C.
