® AN ORDINANCE RENEWING THE INDIVIDUAL AND AGGREGATE STOP
LOSS COVERAGE AND ADMINISTRATION OF THE MEDICAL AND
DENTAL BENEFITS PROGRAM OF THE CITY OF BAYTOWN BROKERED
THROUGH J. E. STONE & ASSOCIATES; AUTHORIZING PAYMENT BY
THE CITY OF BAYTOWN, THE SUM OF THREE HUNDRED THOUSAND
TWO HUNDRED FORTY -TWO AND N01100 DOLLARS ($300,242.00);
MAKING OTHER PROVISIONS RELATED THERETO; AND PROVIDING
FOR THE EFFECTIVE DATE THEREOF.

BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF BAYTOWN,
TEXAS:
Section 1: That the City Council of the City of Baytown
hereby approves renewal of the individual and aggregate stop loss
coverage and administration of the medical and dental benefits
program of the City of Baytown brokered through J. E. Stone &
Associates.
Section 2: The City Council. hereby authorizes payment to the
J. E. Stone & Associates of the sum of ONE HUNDRED FIFTY -EIGHT
THOUSAND ONE HUNDRED SIXTY -ONE AND N01100 DOLLARS ($158,161.00),
for individual and aggregate stop loss coverage.
Section 3: The City Council hereby authorizes payment to
Philadelphia American Life Insurance Company (PALICO) , as third
party administrator of the benefit claims administration, of the
sum of EIGHTY -SEVEN THOUSAND FIVE HUNDRED FORTY -SEVEN AND N01100
DOLLARS ($87,547.00).
Section 4: That City Council hereby authorizes payment to
Cost Care, as a PPO network for individual managed care, of the sum
of FIFTY -FOUR THOUSAND FIVE HUNDRED THIRTY -FOUR AND N01100 DOLLARS
($54,534.00).
Section 5: That pursuant to the provisions of
TEX. L.GOV'T.CODE ANN. § 252.048, the City Manager is hereby granted
general authority to approve any change order involving a decrease
or an increase in costs of FIFTEEN THOUSAND AND N01100 DOLLARS
($15,000.00) or less, subject to the provision that the original
contract price may not be increased by more than twenty -five (25 %)
or decreased by more than twenty -five (25 %) percent without the
consent of the contractor to such decrease.
Section 6: This ordinance shall take effect immediately from
and after its passage by the City Council of the City of Baytown.
INTRODUCED, READ and PASSED by the affirmative vote of the
City Council of the City of Baytown this the 22nd day of September,
1994.
