AN ORDINANCE OF THE CITY COUNCIL OF THE CITY OF BAYTOWN, TEXAS,
AUTHORIZING PAYMENT IN THE AMOUNT OF THREE HUNDRED SEVENTY -
FOUR THOUSAND AND NO /100 DOLLARS ($374,00.00) TO CAREHERE
MANAGEMENT, PLLC, FOR THE ADMINISTRATION OF AN EMPLOYEE
HEALTH CLINIC, INCLUDING MEDICAL SERVICES AND SUPPLIES FOR THE
ONE -YEAR TERM COMMENCING FEBRUARY 5, 2015; MAKING OTHER
PROVISIONS RELATED THERETO; AND PROVIDING FOR THE EFFECTIVE
DATE THEREOF.
BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF BAYTOWN, TEXAS:
Section 1: That the City Council of the City of Baytown, Texas, hereby authorizes payment
to CareHere Management, PLLC, in the amount of THREE HUNDRED SEVENTY -FOUR THOUSAND
AND NO /100 DOLLARS ($374,000.00) for the administration of an employee health clinic, including
medical services and supplies for the one -year term commencing February 5, 2015.
Section 2: That the City Manager is hereby granted general authority to approve any change
order involving a decrease or an increase in costs of FIFTY THOUSAND AND NOI100 DOLLARS
($50,000.00) or less, provided that the original contract price may not be increased by more than twenty -
five percent (25°x'0) or decreased by more than twenty -five percent (25 %) without the consent of CareHere
Management, PLLC, to such decrease.
Section 3: This ordinance shall take effect
City Council of the City of Baytown.
