Ordinance No. 15,707 ORDINANCE NO. 15,707 AN ORDINANCE OF THE CITY COUNCIL OF THE CITY OF BAYTOWN,TEXAS, AUTHORIZING THE CITY TO PAY FRONT LINE MOBILE HEALTH, PLLC FOR ANNUAL MEDICAL EVALUATION SERVICES FOR APPROXIMATELY 158 FIRE DEPARTMENT PERSONNEL IN AN AMOUNT NO TO EXCEED ONE HUNDRED THIRTY-SIX THOUSAND AND NOr 100 DOLLARS ($136,000.00); MAKING OTHER PROVISIONS RELATED THERETO; AND PROVIDING FOR THE EFFECTIVE DATE THEREOF. WHEREAS, Baytown Fire researched firms that could provide annual medical evaluation services for the Fire Department, selected Front Line Mobile Health, PLLC as the vendor following the procurement process, entered into a professional services agreement with Front Line Mobile Health, PLLC, and is now ready to start the evaluations; and WHEREAS,the project is funded at ninety percent(901.o)through the FY-2022 Assistance to Firefighters Grant and the remaining ten percent (105,0') through the Fire Department operating budget; NOW THEREFORE, BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF BAYTOWN,TEXAS: Section 1: That the City Council of the City of Baytown hereby authorizes the payment not to exceed ONE HUNDRED THIRTY-SIX THOUSAND AND NOA00 DOLLARS ($136,000.00) to Front Line Mobile Health, PLLC, for annual medical evaluation services for approximately 158 Fire Department Personnel, payments will be broken down as follows: a. An amount not to exceed ONE HUNDRED TWENTY-TWO THOUSAND FOUR HUNDRED AND NO'100 DOLLARS ($122,400.00) will be paid through the FY- 2022 Assistance to Firefighters Grant; and b. An amount not to exceed THIRTEEN THOUSAND SIX HUNDRED AND N0100 ($13,600.00) will be paid out of the Fire Department operating budget. Section 2: That the City Manager is hereby granted general authority to approve any change order involving a decrease or an increase in costs of FIFTY THOUSAND AND NO.100 DOLLARS ($50,000.00) or less, provided that the original contract price may not be increased by more than twenty-five percent (250-o) or decreased by more than twenty-five percent (25%) without the consent of the contractor to such decrease. Section 3: This ordinance shall take effect immediately from and after its passage by the City Council of the City of Baytown.
