Ordinance No. 15,619 ORDINANCE NO. 15,619 AN ORDINANCE OF THE CITY COUNCIL OF THE CITY OF BAYTOWN, TEXAS, AUTHORIZING AN APPLICATION WITH THE LINCOLN NATIONAL LIFE INSURANCE COMPANY AS THE CARRIER/ADMINISTRATOR FOR THE CITY OF BAYTOWN'S LIFE INSURANCE AND SHORT-TERM AND LONG-TERM DISABILITY BENEFITS;AUTHORIZING PAYMENT BY THE CITY OF BAYTOWN IN AN AMOUNT NOT TO EXCEED SIX HUNDRED SEVENTY-TWO THOUSAND AND NO 100 DOLLARS ($672,000.00); MAKING OTHER PROVISIONS RELATED THERETO; AND PROVIDING FOR THE EFFECTIVE DATE THEREOF. ************************************************************************************ WHEREAS, the City Council of the City of Baytown (the City Council) desires to continue to provide group life insurance, short-term and long-term disability insurance as a benefit to their employees; and WHEREAS, the City Council currently contracts with Lincoln National Life Insurance Company to provide such group benefit plans; and WHEREAS, the City Council finds it in the best interest of the City of Baytown (the City) to continue to offer group insurance benefits to its employees by authorizing the city manager to make an application to Lincoln National Life Insurance Company and negotiate and execute group life insurance plans and short-term and long-term disability insurance benefit plans; WHEREAS, the City Council finds it in the best interest of the City to pay 100 percent of the cost for long-term disability insurance premium payments for their employees and the pass-through cost of employee elected insurance plans; NOW,THEREFORE, BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF BAYTOWN,TEXAS: Section 1: That the City the recitals above are true and correct and made a part of this ordinance. Section 2:That the City Council of the City of Baytown hereby authorizes the payment to Lincoln National Life Insurance Company in an amount not to exceed SIX HUNDRED SEVENTY-TWO THOUSAND AND NO/100 DOLLARS ($672,000.00) for group benefit life insurance, short-term and long-term disability insurance plans. Section 3: That the city manager or his designee is hereby authorized to take any action consistent with this ordinance to affect the terms contained herein. Section 4: That pursuant to the provisions of Texas Local Government Code Annotated Section 252.048, the City Manager is hereby granted general authority to approve any change order involving a decrease or an increase in costs of FIFTY THOUSAND AND NO 100 DOLLARS ($50,000.00) or less, provided that the original contract price may not be increased by more than twenty-five percent (25% or decreased by more than twenty-five percent(25°')without the consent of the contractor to such decrease. Section 5: This ordinance shall take effect immediately from and after its passage by the City Council of the City of Baytown.
